Stale coding docs are worse than none. I’ve seen both Opus 5.5 and GPT-6 Astra trust stale docs over my own code. Switch sessions, models, or agents and your docs go stale before your code does. Run this prompt before any handoff: “Compare every doc in this repo against the actual code. List each claim the code contradicts, with file and line. Don’t fix anything yet. Propose your highest confidence surgical fixes for each.” Approve fixes manually, then hand off.
